Instead of constantly updating your respondents' contact information, have them do the work for you. The Contact Profile gives your audience the opportunity to fill out fields in the Address Book or verify information that’s already there. Whatever’s entered here will be automatically updated once the survey is submitted.

 
NOTE: Don't want the contact profile to display? In the steps below, uncheck every contact and custom contact field, and remove any additional text.

1 Select the contact fields. Begin by selecting your survey. Click Design Survey in the blue navigation bar. By default, the Contact Profile appears first on the Questions page. Click Edit Contact Profile.

Check the contact fields and custom contact fields you would like respondents to enter, and click Next.

2 Configure the Display Options. If you think respondents will need additional help, enter instructions in the Display Text box. Respondents can enter their contact info at the beginning or end of the survey depending on what you select for "Display contact fields."

Use the textboxes in the Display Name column to rename the field. Under the Required column, determine if the respondent will need to fill out the field before they can move on to the next page. Rearrange the order of the fields by clicking the arrows under the Move column.

Click Finish. If you have decided to display the Contact Profile at the end of your survey, the Contact Profile will automatically move to below your questions.
